Hey there! Welcome to Only Fans Insider! We’re excited to have you here. For our readers who might not know you yet, can you tell us a little about yourself and the business you’ve built in the OnlyFans world?

Thanks so much for having me! I’m Dr. Don Revis — but most people know me as “America’s Breast Surgeon” or “The Boob Whisperer.” I’m a Board-Certified Plastic Surgeon, and for the past 25+ years, I’ve dedicated my entire practice to one thing and one thing only: breast augmentation.

Very early in my career, I realized I wanted to go all in on this specialty. I developed a technique called the Original Internal Bra, which provides long-term lift and support by using a strong, permanent internal suturing method. It’s become my signature procedure — one that I’ve trademarked, taught to other surgeons, and refined over decades.

What makes my practice unique is that it’s truly international. Over 80% of my patients fly in from out of state or abroad, so my team and I have built a seamless process for both in-person and virtual consultations. Whether someone wants a subtle, natural look or something more dramatic, we tailor everything to their goals.

And honestly, if you spend any time on OnlyFans, you’ve probably seen my work without realizing it. Many top creators and performers have trusted me with their transformations — people like Cory Chase, Gigi Dior, Lexi Luna, Christy Mack, Elle Matthews, Lynnie Marie, Kitana Montana, and thousands more. It’s been an incredible honor to help so many remarkable women feel confident in their own skin and their careers.

At the end of the day, this is what I love doing — creating results that look and feel amazing, and helping my patients feel proud, empowered, and camera-ready.

We’re always curious—was your business a planned venture, or did it evolve naturally over time?

I always knew I wanted to be a plastic surgeon, but my specialization in breast augmentation really didn’t take form until I opened my own solo private practice in 2000 in Fort Lauderdale. There was so much mediocre and frankly bad plastic surgery going on that I knew I had to come up with a solution to fix all of the problems that kept walking in the door.

I came up with the Original Internal Bra- my strong permanent internal suturing technique I use to repair a host of problems- bottoming out, lateral displacement, synmastia, etc. Over the years I successfully obtained a U.S. Trademark for the procedure, and now both patients and plastic surgeons from around the world inquire about my technique and its success. Just in the past week I’ve spoken to three plastic surgeons from three different European countries about the technique and its nuances and how I apply it in certain patient situations.

I also don’t pay for any marketing or advertising- I have relied on word of mouth, online reviews and social media to spread the word organically. In doing so, now I have over 250,000 followers on Instagram and over 75,000 unique visitors to my website each month.

Young plastic surgeons often come to me for advice and it’s very simple- work hard, be honest with patients, treat them fairly, identify problems with current techniques and come up with solutions, and hone your surgical talents to the best of your ability. There are no shortcuts in this business- the 16 years of college, medical school and residency are only the beginning, a downpayment so to speak- once you enter private practice is when the real hard work begins and it takes many years to become the best and to be recognized for it.

It took at least the first ten years to grow my business and become internationally recognized as a leader in the field of breast augmentation before I had a constant stream of patients and even had to create a waiting list. Now we greet patients from around the world- all 50 states and over 78 foreign countries. My waiting time for surgery is about three to four months and I perform over 750 breast augmentations a year.

Although I had the opportunity to build out an operating room in my office, like so many other plastic surgeons have done to generate more income, I always felt surgery should be taken more seriously and would be safer performed in a full service accredited hospital facility. I choose to operate in a fully equipped and certified Ambulatory Surgery Center (ASC), run by a local hospital, with their board certified M.D. anesthesiologists and nursing staff. I operate there Mondays, Wednesdays and Fridays and typically perform 5-6 breast augmentation surgeries on each of those days.

On Tuesdays and Thursdays I see patients in my office for consultations, preop and postop visits. I keep my staff very small- no extraneous spa or skin care practice to create clutter and distractions- just 100% breast augmentation surgery all day, every day. I think that helps us focus and provide the best possible care and results, consistently, day after day, year after year.

What keeps you motivated to innovate and grow your business within this ecosystem?

Twenty-five years in practice, and over 17,000 breast augmentations later, I still can’t wait to go to work every morning. I love my job- and I was made for this!

Breast augmentation is a fascinating surgery, and every single patient is different. Whether it’s a first time augmentation patient who wants to go from an A cup to a C cup, or someone who has had multiple surgeries with complications, every patient presents a unique set of challenges and has a unique set of goals I must meet.

And patients have very different requests- some want small natural soft breasts, while other want “high round and hard.” What’s critical is to escape the mindset that “one size fits all” when approaching breast augmentation.

Too many plastic surgeons think breast augmentation is a simple procedure that you always do the same way with the same implants. Nothing could be further from the truth- individualizing each patients treatment, procedure, and implant selection based on their desired look is the key to success.

Every business has its little secrets—what’s one of yours that would surprise your audience?

When I stopped doing other plastic surgeries (i.e. facelifts and tummy tucks) about ten years ago- my office became much more efficient and streamlined and I became a better surgeon because I could focus all of my energy and attention in one direction- breast augmentation.

This also allowed me more time to direct my personal attention to my branding. I designed my own logo, through dozens of redesigns and colors, until I was finally satisfied that it represented me and my practice.

Regarding my website, I wrote every single word of it myself, and I selected every single photo myself. When it comes to social media, I run my own account. I want everything presented to the public to represent my own personal style- it’s my reputation on the line, and my personal attention makes sure it’s 100% authentic.

I’m also working on succession plans. I’m 58 years old and will probably practice for another 15 years. Prior to retirement, I plan to select a young plastic surgeon to join my practice and learn my techniques so that they can step into my shoes and provide a consistency of care so that all I have learned and created is not lost when I step away. I want the Original Internal Bra to continue to provide excellence in breast augmentation long after I’m gone.

In your opinion, what’s next for creators, agencies, and businesses working in this space?

I think we are likely to see an improvement in implant technology moving forward. The two most common complications of breast augmentation- capsular contracture and implant rupture- have been decreasing with improvements in implant material and will likely continue to improve.

Twenty years ago silicone implants were filled with liquid silicone and implants, both saline and silicone, had a tendency to break, hence the common saying that every ten years you had to replace them. Silicone implants are no longer filled with liquid silicone, but a more cohesive gel, which doesn’t spill if broken. Saline implants are much more durable. The “every ten year” rule is no longer valid. If your implants look good and feel good and you’re happy with them, there is no reason to replace them.
